NURS 8500/8500A/8500F/8500M/8500S/8500W/8500C: Evidence-Based Practice II: Planning and Implementation | Week 1

In this complex and rapidly evolving world, successful organizations and those who work within them must be able to embrace the need for change and be adaptable to new developments. As a DNP student and a nurse with advanced specialty area skills, you are engaged in the translation of evidence into practice—a process that generates change, which can be exciting and challenging for everyone involved. As a nurse leader, you have the opportunity to inspire and guide others through this process. In particular, you can play a pivotal role in promoting well-planned change efforts that further your profession and the broader field of health care.

What does it take to promote a successful change effort? Why do many initiatives or new practices fail to be adopted and fully implemented? In this course, you will hone the knowledge and skills necessary to effectively plan and implement change at the aggregate level.

This course provides support for your practicum experience, which you also engaged in during NURS 8400 and NURS 8410. You will look at systems-related issues that contribute to challenges for an organization or other aggregate; for instance, what might inhibit patient flow from the ER to admitting to placement in an acute care unit? Throughout this course, you will be able to share strategies and ask for support from your colleagues and Instructor for the development and implementation of your project.

This week, you develop practicum objectives that align with the AACN Essentials and the professional standards from your area of specialization. You also create an overview or visual representation of an approach for planning and implementing change related to the translation of evidence into practice. This includes attention to the vital function of leadership in such endeavors.

Learning Objectives

Students will:

  • Analyze approaches of planning for and implementing change
  • Analyze strategies to translate evidence into practice
  • Analyze the influence of practicum experiences on professional growth
  • Develop practicum objectives aligned to the AACN Essentials, professional standards, and specialty guidelines
  • Evaluate approaches for planning for and implementing change related to the translation of evidence into practice

“I am personally convinced that one person can be a change catalyst, a ‘transformer’ in any situation, any organization. Such an individual is yeast that can leaven an entire loaf. It requires vision, initiative, patience, respect, persistence, courage, and faith to be a transforming leader” (Covey, 2009, p. 287).

Note: Before you begin this week’s Discussion, please be sure to post your introduction in the Class Café.

Your practicum experiences in NURS 8400 and NURS 8410 have provided opportunities to gain knowledge, perspective, and skills that bolster your specialty area practice. They have laid a foundation for reflecting on your own strengths and challenges as a nurse leader in a dynamic workplace.

In this course, you continue to engage in your practicum experience, with specific attention to systems-related issues that contribute to challenges at the aggregate level, the change process, and leadership. This Discussion serves as a springboard for the practicum assignment you are to develop this week. Be sure to view and consider the instructions for this week’s practicum assignment in preparation for the Discussion.

To prepare:

  • Reflect on your practicum experiences in NURS 8400 and NURS 8410. Drawing on your knowledge of systems thinking, what problems have you noticed in your practicum setting that may be related to systems issues?
  • How would you outline a viable approach to planning and implementing change to address a systems-related change? What are the essential steps and focal points from beginning to end? What is the role of leadership when it comes to change efforts? Think about the information presented in the Learning Resources, and consider insights you have gained that may be useful as you explore the process of intentional change. Conduct additional research using current literature to further your analysis.
  • Assess the professional growth (including leadership development) you have attained through your practicum experiences during previous courses in the DNP program.
  • Begin to develop your plan for the practicum, including two or three learning objectives.

By Day 3

Post a cohesive response that addresses the following:

  • Describe key aspects of your suggested approach to planning for and implementing change. What are two or three essential steps or strategies that deserve particular attention as you strive to successfully translate evidence into practice? Provide your rationale.
  • Assess the influence of your practicum experiences on your professional growth. Identify areas for improvement to focus on during this course. Given what you have learned in previous courses and through your professional experiences, what questions or concerns do you have regarding how to identify and assess systems-related issues, as well as how to plan for and implement change?
  • Summarize your planned practicum experiences and how you expect these experiences to advance your professional skills. Support your choices with documented evidence from the AACN Essentials, professional standards, and specialty guidelines.

Read a selection of your colleagues’ responses.

By Day 6

Respond to at least two of your colleagues in one or more of the following ways:

  • Ask a probing question, substantiated with additional background information, evidence, or research.
  • Share an insight from having read your colleagues’ postings, synthesizing the information to provide new perspectives.
  • Offer and support an alternative perspective using readings from the classroom and/or from your own research in the Walden Library.
  • Validate an idea with your own experience and additional research.
  • Make a suggestion based on additional evidence drawn from readings or after synthesizing multiple postings.
  • Expand on your colleagues’ postings by providing additional insights or contrasting perspectives based on readings and evidence.

NURS 8500/8500A/8500F/8500M/8500S/8500W/8500C: Evidence-Based Practice II: Planning and Implementation | Weeks 2 and 3

“Truly the art of leadership, like any art, requires the discipline of practice and commitment to continued professional growth. As must a pianist play scales endlessly so that Mozart’s music could someday flow from her fingertips, so must a leader follow principles of great stewardship everyday to make her leadership seem endless,” (Beeman in Dye & Garman, 2006, page xi).

In previous courses, you have explored theory related to leadership and the initiation and management of change. During the practicum experience for this course, you are honing germane skills by engaging in leadership activities related to a change that involves the translation of evidence into practice. Over the next several weeks, you will explore a series of topics related to planning for and implementing evidence-based change. The presentation of topics is not intended to be comprehensive or sequential; however, your examination of these topics should support and enrich your practicum experiences.

Now, in Weeks 2 and 3, you are asked to consider examples of leadership and to assess your own leadership competencies. Working with individuals in your practicum setting, you examine examples of changes that are currently being made—or changes that have already been achieved. You also critically analyze the environment and assess data to identify practices or situations that are ineffective or could benefit from improvement. From this appraisal, you propose an evidence-based change at the aggregate level. You will use this proposed change as a means of considering critical aspects of planning and implementation as you proceed through this course.

Learning Objectives

Students will:

  • Analyze the influence of leadership on the translation of evidence into practice
  • Evaluate the practicum setting to propose an evidence-based change
  • Assess leadership skills in relation to the practicum experience
  • Assess the practicum experience

What examples of leadership have you seen in working with others at your practicum setting as well as in other situations? What have you observed that inspires you to be an effective leader? What challenges have you noticed? How does this relate to your personal leadership style? How can you, as a leader, facilitate the translation of evidence into practice? What leadership skills aid this process?

For this Discussion reflect on what makes a leader effective and how insights from theory as well as previous endeavors can be applied to planning for and implementing a change related to the translation of evidence into practice.

To prepare:

  • Consider evidence-based changes currently underway or recently achieved in your practicum setting. Analyze the practicum environment, examine data, and refer to theory to identify a need for a change you would propose.
  • Reflect on examples of how leadership influences the translation of evidence into practice from within your practicum setting.
  • Identify an evidence-based change appropriate for your practicum setting.
  • Think of two or three instances that demonstrate how you have been able to—or, if not applicable, how you could—apply leadership skills in your practicum setting. Then, reflect on the results of your DiSC Assessment, completed in NURS 8300. What strengths and/or challenges have you encountered or might you encounter based on your personal leadership style? In what ways do you need to grow as a leader?

By Day 3 of Week 2

Post a cohesive response that addresses the following:

  • Based on your practicum experiences, discuss the influence of leadership on the translation of evidence into practice. Provide specific insights gained from theory as well as from changes currently underway or recently achieved in your practicum setting.
  • Propose an evidence-based change for your practicum setting. Be sure to justify the need for this change using data.
  • Explain how you might apply theory as well as observations gathered from your practicum site to planning for and implementing the proposed change.
  • How are you developing your leadership skills in your practicum setting? Which aptitudes or experiences do you think are most important to your role as a leader in facilitating the translation of evidence into practice? If possible, relate this to specific learning objectives you developed during Week 1.

Read a selection of your colleagues’ responses.

By Day 3 of Week 3

Respond to at least two of your colleagues in one or more of the following ways:

  • Ask a probing question, substantiated with additional background information, evidence, or research.
  • Share an insight from having read your colleagues’ postings, synthesizing the information to provide new perspectives.
  • Offer and support an alternative perspective using readings from the classroom and/or from your own research in the Walden Library.
  • Validate an idea with your own experience and additional research.
  • Make a suggestion based on additional evidence drawn from readings or after synthesizing multiple postings.

NURS 8500/8500A/8500F/8500M/8500S/8500W/8500C: Evidence-Based Practice II: Planning and Implementation | Weeks 4 and 5

A key aspect of any successful change initiative is identifying who will be involved in and affected by the change. How do you generate interest and investment in the change you are proposing? How do you facilitate the development of new skills and dispositions required to adopt—and ultimately sustain—the change?

As a nurse leader, it is important to develop the awareness and political acumen necessary to work with both the formal and informal structures of power and authority within a particular setting. You must also be able to recognize which groups may require education and training in order to be able to adopt and implement a change related to the translation of evidence into practice.

In Weeks 4 and 5, you consider issues surrounding communication and knowledge integration as they relate to a proposed evidence-based change in your practicum setting.

Learning Objectives

Students will:

  • Propose strategies related to communication and education for implementing evidence-based practice
  • Evaluate potential challenges for knowledge integration

Who provides formal and informal leadership within your practicum setting? What power and/or authority do these individuals hold? How has this influenced planning for and implementing change in the past?

In this Discussion, you analyze the dynamics of your practicum setting with attention toward developing political acumen, which can be highly beneficial in your effort to facilitate evidence-based change. You also consider the need for education and methods that could be used to promote knowledge integration among various stakeholders. As you probe these issues, your goal is to develop strategies for building alliances that would support the successful implementation of the change you are proposing for your practicum experience.

To prepare:

  • How would you describe the distribution of formal and informal power, as well as authority, within your practicum setting? Think about the constellation of professional roles. Which ones are most salient for the change you are proposing? Who holds each of these roles? Who else within the organization might have political clout and be able to serve as an ally? What approach would you take to build productive alliances and generate buy-in for your proposed change?
  • How do you anticipate that this change would cascade through the environment? What strategies would you employ to generate wide-scale interest and investment in your proposed change, as well as the education necessary for successful adoption? Evaluate specific structures related to communication, training, organizational culture, and other factors that relate to the implementation of sustainable change.
  • What kinds of issues related to organizational structure and culture or interpersonal dynamics could pose a challenge for creating buy-in and/or promoting knowledge integration?
  • In addition to reviewing this week’s Learning Resources, be sure to speak with individuals in your practicum setting and conduct additional research to enrich your thinking for this Discussion.

By Day 3 of Week 4

Post a cohesive response that addresses the following:

  • Based on the specifics of your practicum experience and evidence from the literature, what strategies would you employ to communicate with and educate various stakeholders? How would you engage the support of those with power and authority? Explain how this would facilitate the successful implementation of your proposed evidence-based change.
  • What are potential challenges for knowledge integration? How might you address those challenges?

Read a selection of your colleagues’ responses.

By Day 3 of Week 5

NURS 8500/8500A/8500F/8500M/8500S/8500W/8500C: Evidence-Based Practice II: Planning and Implementation | Weeks 6 and 7

The allocation of resources often reflects organizational priorities and commitments that have already been made. Unfortunately, resource constraints are prevalent in most organizations. As a nurse leader, it is essential to understand how a proposed change may be affected by—or may affect—the allocation of resources within a setting. Financial implications, for instance, can be a key facilitator or inhibitor of a proposed change. For example, finances and other resources may be committed to an initiative at the outset, and then need to be reassessed due to developments within and outside of the setting.

Often, one of the most significant—and expensive—resources in an organization is the health information system. This week, you consider the use of health information systems and other resources to support the integration of evidence-based change.

Learning Objectives

Students will:

  • Evaluate use of health information systems in the practicum setting to support evidence-based change
  • Assess leadership development in the practicum setting

For this Discussion, you are asked to evaluate the use of a health information system and other resources in your practicum setting. What are the implications of resource use in terms of planning for and implementing your proposed evidence-based change?

To prepare:

  • Consider the benefits and limitations of the use of a health information system within your practicum setting. What impact do you think this might have for your proposed evidence-based change? Draw upon at least one relevant example of a current or previous change initiative at your setting as you assess this. (If your setting does not have a health information system, you can still discuss the benefits and limitations of not having a health information system as it relates to this change.)
  • What other resources (e.g., financial, staffing, materials) are important to consider as well? Why?
  • Be prepared to support your Discussion postings with evidence from the literature and specific examples related to your practicum setting.

By Day 6 of Week 6

Post a cohesive response that addresses the following:

  • Within your practicum setting, what are the benefits and limitations of using a health information system? How might this impact planning for and implementing your proposed change?
  • Describe three potential resource considerations that could facilitate or inhibit your proposed change and explain your rationale.

Read a selection of your colleagues’ responses.

By Day 3 of Week 7

Respond to at least one of your colleagues in one or more of the following ways:

  • Ask a probing question, substantiated with additional background information, evidence, or research.
  • Share an insight from having read your colleagues’ postings, synthesizing the information to provide new perspectives.
  • Offer and support an alternative perspective using readings from the classroom and/or from your own research in the Walden Library.
  • Validate an idea with your own experience and additional research.
  • Make a suggestion based on additional evidence drawn from readings or after synthesizing multiple postings.
  • Expand on your colleagues’ postings by providing additional insights or contrasting perspectives based on readings and evidence.

Return to this Discussion in a few days to read the responses to your initial posting. Note what you learned and/or any insights you gained as a result of the comments made by your colleagues.

Be sure to support your work with specific citations from this week’s Learning Resources and any additional sources.

NURS 8500/8500A/8500F/8500M/8500S/8500W/8500C: Evidence-Based Practice II: Planning and Implementation | Weeks 8 and 9

As a nurse leader, your involvement in the translation of evidence into practice includes developing outcomes that will be used to evaluate a change initiative. As you have considered in previous courses, evaluation must be integrated into planning from the very beginning, and implementation should be aligned to produce the required evidence. Collaboration with other individuals within your profession and across other disciplines can help you to develop outcomes that are relevant and useful for the organization as a whole.

This week, you develop measureable outcomes to guide planning for and implementation of your proposed change related to the translation of evidence to practice in your practicum setting. This exercise provides a valuable opportunity to look at your proposed change from multiple viewpoints. Have you considered the potential impact of this change for all groups? Looking across the complex health care landscape, which outcomes would be most beneficial for evaluating the success of the endeavor? How will you, as a leader, establish accountability around your proposed change?

Learning Objectives

Students will:

  • Analyze resources to develop measureable outcomes for evidence-based change
  • Analyze measureable evaluation criteria for evidence-based change outcomes
  • Assess the importance of intra- and interdisciplinary collaboration for the design of outcomes

As noted in previous courses of the DNP program, it is essential to design measureable outcomes that can guide the development, implementation, and evaluation of an initiative. For this Discussion, you are asked to examine professional standards and other factors that require consideration as you develop outcomes for your proposed evidence-based change. Think about the various groups that are invested in—or that may be affected by—this change. What measures would stakeholders need in order to help determine if the change aligns with organizational priorities and is successful in achieving its intended outcomes? Why is it important to establish accountability around this initiative?

To prepare:

  • Look at outcomes established for at least one evidence-based change currently underway or previously accomplished within your practicum setting. What lessons can be learned to support the development of outcomes for your proposed change?
  • What outcomes would you identify for your proposed change? How are these tied to AACN Essentials? What other standards or guidelines should be considered? What interdisciplinary factors need to be considered as you craft these outcomes? Think about other clinical and administrative groups, as well as patients.
  • How will the outcomes be measured?
  • Be prepared to support your Discussion postings with evidence from the literature and specific examples related to your practicum setting.

By Day 3 of Week 8

Post a cohesive response that addresses the following:

  • Describe two or three outcomes for your proposed change. For each outcome, describe the evaluation criterion/criteria that is/are measurable.
  • Explain how these outcomes relate to multiple sets of standards, as well as differing groups’ needs and priorities. Why is intra- and interdisciplinary collaboration important as you design these outcomes?
  • How do the measurable outcomes for your proposed change establish accountability for you as a leader?

Read a selection of your colleagues’ responses.

By Day 3 of Week 9

NURS 8500/8500A/8500F/8500M/8500S/8500W/8500C: Evidence-Based Practice II: Planning and Implementation | Weeks 10 and 11

During Weeks 10 and 11, the attention turns to the multiple systems that affect and are affected by an intended change. Consider the evidence-based change you are proposing for your practicum setting: How would you describe the various systems or components of a system that may be impacted by this change? For instance, if your change initiative is targeted to a specific health care organization, what implications could it have for a particular patient population, or for other organizations or professional groups at the local, state, national, or global levels?

In addition, you consider how other changes within the aggregate or outside of it may influence the implementation and sustainability of your change initiative. Paying attention to these types of evolving issues during planning and throughout implementation can help you prepare for the opportunities and challenges you are likely to encounter as a nurse leader.

Finally, this course has addressed critical issues related to the translation of evidence into practice and the opportunities and challenges associated with change. As this course comes to a close, reflect on your professional and academic development through the practicum. How have you seen yourself grow as a nurse leader? How has your work on the project progressed during the previous weeks? In this week’s Discussion, you will apply the insights you have gained from your participation in this course to strategies for your continued development as a nurse leader.

Learning Objectives

Students will:

  • Analyze the effects of evidence-based change on aggregates at various system levels
  • Assess the practicum experience
  • Assess professional growth related to practicum
  • Evaluate strategies to promote professional growth

As an advance practice nurse, your work is informed by many factors beyond your direct reach, and your influence extends beyond the immediate context in which you labor. Likewise, the changes you propose as a nurse leader are influenced by multiple systems and, in turn, can have an impact on many different groups. One of the ways to distinguish yourself as a leader is to be knowledgeable about these spheres of influence and to promote awareness of the potential effects that may result from a change.

Indeed, with the fluid and persistently evolving nature of change, it is essential that you stay abreast of latest developments that may affect the practice environment. This is especially critical for maintaining or adapting a change once it has been implemented. As discussed in Weeks 8 and 9, you may need to make adjustments based on evaluation of outcomes; in addition, you must also be prepared for adjustments based on developments within or outside your targeted environment.

To prepare:

  • What are some possible adjustments that you might need to make based on the evaluation of outcomes? Consider insights from the Discussion in Weeks 8 and 9.
  • Consider the evidence-based change you are proposing for your practicum setting: What other systems or components of a system are within the sphere of influence of this change? How could evolving issues internal to the setting, such as budget, staffing shortage, regulations or external to it, such as supply, funding, national priorities, and disasters, affect the implementation—and ultimately the sustainability—of such a change?
  • How might your proposed change, in turn, affect policy or practice at other levels beyond your initial focus?

By Day 3 of Week 10

Post a cohesive response that addresses the following:

  • Describe at least three evolving issues that are internal or external to your practicum setting that could affect the implementation and sustainability of your proposed change. From what systems level do those issues originate (e.g., organizational, local, state, national, global) and what other systems are then involved in response to that factor?
  • How might your proposed change, in turn, lead to shifts at other levels beyond your initial focus?

Read a selection of your colleagues’ responses.

By Day 3 of Week 11

Return to this Discussion in a few days to read the responses to your initial posting. Note what you learned and/or any insights you gained as a result of the comments made by your colleagues.
